This is a co-design starting point, not a final assessment tool. Schools should select and review questions with pupils, safeguarding staff and relevant pastoral/SEND professionals.

## Avoid
- Leading or accusatory wording
- Asking pupils to repeatedly disclose traumatic detail
- Promising confidentiality that the school cannot provide
- Turning the check-in into a behaviour score
- Using one response as evidence of a diagnosis
